By keeping our patio area clean, consistent and clear on Sundays, we will make our ministering efforts user-friendly and effective. By completing this form, we are working together to accomplish our common goals.

    This form must be submitted 2-weeks prior to the date that you want to have your table.

      Please list the date(s) you would like a table on the patio. (Maximum of 4 weeks)

      Please list the names of the volunteer staff that will be manning the table for each time slot on each date:

      NewCov will be creating 11" x 17" signage for your table. Please let us know what you want your sign to say.

      You will need to provide us a sample of the materials that you will be distributing for approval. You can accomplish in one of two ways:

      • Submit your materials via email to Gayle Janzen at [email protected].

      • You can also leave a copy for Gayle at the Welcome Center on Sundays.

      This step must be completed before your request for a table will be approved.
